PERTAINS TO:   All Full Time Employees

 

[Insert Company Name] will grant all full time employees bereavement leave of [Insert number] days off, with pay, in the case of the death of an immediate family member.   Such leave must be authorized by the employee´s department head/supervisor only.   An immediate family member is defined as:

·               [List Below Members Who Will Be Included in the Company´s Bereavement Policy]

 

Upon the employee´s request, an additional [Insert number] days may be granted, which must be approved by the employee´s immediate department head/supervisor.   If accrued sick or vacation leave is available, this benefit will be used for the additional [Insert number] days; otherwise the additional [Insert number] days will be unpaid.  

 

Bereavement leave for death of individuals other than the immediate family members listed above must be approved by the employee´s immediate department head/supervisor.   Time off for such an instance is limited to [Insert number] days and will be unpaid.
