Best practices for phone interviews

My company is interested in revamping our interviewing processes by including phone interviews as preliminary interviews to weed out applicants not suited for our industry. Our recruiters do basic phone interviews with a script but we are interested in adding more in depth interviews with a manager and HR. What are some of the questions and practices you use for successful and informative phone interviews?
